Bueno, revisé tu código, el error principal que puedo ver es ese error de definición múltiple.
Causa principal:
La razón principal de esto es que estás cargando esas bibliotecas más de una vez que exponen define.
Solución:
En su caso, noté que ha cargado ArcGIS JS Api más de una vez.
Solo cargue una sola fuente de ArcGIS, debería funcionar.
Una más delgada en el código adjunto que está utilizando múltiples versiones de CSS.
- se supone que debes usar la misma versión de ESRI CSS que estás usando para JS.
- De acuerdo con su código, está utilizando un modelo heredado de estilo de codificación, así que

.
