Solicitud de SOAP en PHP con CURL

Como el manual de SOAP en php.net no es muy amigable con los novatos y no pude encontrar ningún buen ejemplo, publicaré mi pregunta aquí.

¿Cómo puedo crear una solicitud PHP SOAP para que se vea así?
